First Visit

Prior to your check-up, we will ask you to share background information about your general health, medications, previous surgeries and dental procedures, lifestyle issues, and more. An extensive medical and dental history is extremely important as this information may be connected to your oral health.

To save yourself some time, you can download our new patient forms in advance and bring them into the office on your first visit.

At the time of your consultation, we ask you to provide:

  • Your referral slip and any x-rays if applicable.
  • A list of medications you are currently taking (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
  • If you have medical or dental insurance, please bring the necessary completed Patient Registration forms. This will save time and allow us to help you process any claims.

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please note that all patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
